Family members’ applicationsIf the employee is intending to apply for his/her family as well, he/she must submit the following documents on behalf of each family member:
In addition, it is very important to know that family members over the age of 14 need to arrive at the embassy for the interview in order to submit fingerprints. Electronic Visa Application Form (EVAF) procedure:The EVAF form is available at http://EVISAFORMS.STATE.GOV. It is crucial that the EVAF form is filled out correctly and that you receive a barcode on the third page. After accessing this link, you may fill out the form by clicking in the DS-156 application form. After you have completed the form, please press a continue button (if you do not press the button, you will not get the third page with the bar code). The form is then displayed in Adobe Acrobat where it can be printed. The first two pages will be a completed DS-156, to which you will add signature and photo. The third page will contain the 2-bar code which will enable scanning at the Embassy. Any Inkjet or laser printer should work for this purpose, but a dot matrix printer would not print as sufficient resolution. Additional instructions are also found on the web site. All other visa forms can be downloaded from the State Department website, http://foia.state.gov/FORMS/newscripts/formlist.asp?type=V.
