Employee’s E visa applicationEach file must include an application on behalf of the employee the company is interested in relocating to the U.S. Applicants must submit the following forms and documents to support their claim to an E status. Such documents must convince the consular officer that the applicant is essential to the operation of the company in the U.S. The applicant needs to submit the following documents in order to receive a visa:
Electronic Visa Application Form (EVAF) procedure:The EVAF form is available at http://EVISAFORMS.STATE.GOV. It is crucial that the EVAF form is filled out correctly and that you receive a barcode on the third page. After accessing this link, you may fill out the form by clicking in the DS-156 application form. After you have completed the form, please press a continue button (if you do not press the button, you will not get the third page with the bar code). The form is then displayed in Adobe Acrobat where it can be printed. The first two pages will be a completed DS-156, to which you will add signature and photo. The third page will contain the 2-bar code which will enable scanning at the Embassy. Any Inkjet or laser printer should work for this purpose, but a dot matrix printer would not print as sufficient resolution. Additional instructions are also found on the web site.
