Downloading Visa forms
Public use visa forms on this Internet site are now provided in a format that can be filled out on your computer. In order to use the fillable features, please note the following:
- You cannot submit the application on the Internet. Rather, these fillable forms will enable you to fill out the form, and print it out - all on your local computer.
- The forms available for downloading (printing) from this Website are the approved versions. Only approved forms will be accepted by the Department of State, Embassy or Consulate, and forms that do not comply with these specifications will not be accepted.
- You will not be able to save this file on your own computer. We recommend that you fill out a page of the form, then print it prior to quitting Acrobat Reader.
