Tel Aviv Federal Benefits Unit will no longer be accepting SS-5’s (Social Security Number applications) from people who reside outside of our consular district, as of April 1, 2008. If you reside in the municipality of Jerusalem, the West Bank or Gaza, please apply at the U.S. Consulate General Jerusalem (http://jerusalem.usconsulate.gov).

  FBU Summer 2008 hours  from July 1, 2008 through September 30, 2008:

• Walk-in service hours: Tuesday, Wednesday and Friday, 9-11 am, except the last Friday of the month.
  
• Telephone inquiries:  Thursday, 9-11 am.

Please Note:  In addition, FBU will be CLOSED to the public July 23 through July 29.  This includes walk in service as well as all inquiries.

Beginning on February 1, 2008, US regulations will require that both parents be present to sign the passport application or visa pages for all minors under the age of 16. This replaces the current regulations which allow minors to sign for themselves at the age of 14.

After February 1, a complete passport application for a minor under the age of 16 will consist of the passport application, 2 color photos sized 5cm X 5cm, the original (or certified true copy) of the minor's birth certificate or Consular Report of Birth Abroad Certificate, and the minor's previous US passport (if applicable). Of course the child and both parents need to be present. If both parents cannot be present, we can accept the following documents as proof of consent from the other parent:

• A notarized consent Form DS-3053 from the other parent. The consent must include a copy of the U.S. or foreign passport of the parent who is not appearing at the Embassy;
• Original Death Certificate;
• Original Court order from granting “sole-custody” (bilahdit) to the parent appearing at the Embassy; or
• Original Birth certificate listing only one parent.
