Passport Renewal For –
APPLICANTS AGE 15 AND UNDER
Please follow the instructions on this page if:
- You are under the age of 16; and
- You are able to submit your most recent passport with your application. (If you are not able to submit your most recent passport, please go to Lost or Stolen Passports).
Application Requirements
Please bring the following to the Embassy for your appointment:
1) Your most recent United States passport;
2) Orginal or certified copies of documents including all passports and proof of citizenship.
3) Any foreign passport that you may currently hold, have held, or been included in;
4) Name change documentation:
- If you have changed your name since your most recent passport was issued, you will be required to furnish name change documentation. Please follow this link for additional application requirements;
5) Completed Passport application form DS-11:
- The instructions on pages one and two do not apply if you are applying for a passport in Israel. Follow the instructions on this web page. Do NOT print Form DS-11 on front and back of the same sheet. For tips on completing the DS-11, please follow this link.
6) Two identical passport 5x5 centimeter photographs:
- The Embassy will reject any photographs that do not meet the specified requirements. Vending machine photographs and photographs printed on a home computer are unacceptable. Follow this link to photo requirement specifications;
7) Fees: The application fee for an applicant under the age of 16 is U.S. $85.00; aged 16 and over is $100.00. Accepted payment methods are:
- Cash - either dollars or shekels; or
- Most international credit cards; or
- Travelers Checks in U.S. Dollars.
Note: The Embassy is not authorized to accept personal checks.
8) Envelope:
- New passports must be sent back to applicants once they are processed. In order to receive your new passports, you must bring, either:

9) All children are required to appear at the Embassy when applying for their passports.
10) Both parents of children are required to appear at the Embassy when applying for passports for children under the age of 16.
- If only one parent is able to attend, we can accept the following documents as proof from the other parent:
- A notarized consent Form DS-3053 from the other parent. The consent must include a copy of the U.S. or foreign passport of the parent who is not appearing at the Embassy;
- Original Death Certificate;
- Original Court order from granting “sole-custody” (bilahdit) to the parent appearing at the Embassy; or
- Original Birth certificate listing only one parent.
11) Parent’s U.S. or Israeli passports.
12) Child’s Birth Certificate.
- An original U.S., Consular Report of Birth or foreign birth certificate which contains both parents’ names is required. Abstracts of birth, also known as birth abstracts, and hospital birth certificates are NOT accepted.
Note: If the name of the parent in the passport is not the same as the name used for the parent on the birth certificate, the parent must submit documentation that will link both names or link the parent to the child (marriage/divorce certificate, court order, official name change document);
13) If your previous passport was issued when you were a small child, you may need to bring pictures showing your physical development since the previous passport picture was taken, and/or other proof of identity.
14) Additional documents may be required depending upon the circumstances of your case.
Processing Time
An application for a full-validity (five years for children) passport will be sent to the United States for printing with a secure, photo-digitized image of the bearer. Passport times vary depending on workload. Please allow 3-4 weeks for the application to be processed. |
