first time Passport application
Please follow the instructions on this page if:
- You have never had a U.S. passport issued in your own name; and
- You have been issued one of the following documents (If you are no longer in possession of the original document, you must obtain a certified copy from the issuing authority before submitting your application):
- A U.S. Birth Certificate – please follow this link for information on obtaining certified copies;
- A U.S. Consular Report of Birth Abroad (Form FS-240) – please follow this link for information on obtaining certificated copies;
- A U.S. Certification of Birth (Form FS-545 or DS-1350) - please follow this link for information on obtaining certified copies;
- A U.S. Certificate of Citizenship - please follow this link for information on obtaining certified copies;
- A U.S. Naturalization Certificate - please follow this link for information on obtaining certified copies;
- A passport of your U.S. citizen parent(s) in which you are included - please follow this link for information on obtaining certified copies.
Note: The parents of an American citizen children born outside of the United States must report the child's birth to the Embassy or Consulate General. You may apply for the child's first passport at the same time you make the report of birth. Please be aware that you must have a separate appointment for each of your children for whom you are making a report of birth and/or requesting a passport. For example, if you have four children and want to report all of the births at the same time and want also to apply for their passports, your family will need four appointments -- one for each child. You can make these appointments on the same or different days. If you need to make seven or more appointments for you children and want them to be at the same time, please email us at amctelaviv@state.gov for a special appointment.
Application Requirements
Please bring the following to the Embassy for your appointment:
1) Proof of U.S. citizenship:
- an original U.S., Consular Report of Birth or foreign birth certificate which contains both parents’ names (abstracts of birth, also known as birth abstracts, and hospital birth certificates are NOT accepted); or
- a U.S. Consular Report of Birth Abroad; or
- a U.S. Certification of Birth; or
- a U.S. Certificate of Citizenship; or
- a U.S. Naturalization Certificate; or
- a passport of your U.S. citizen parent(s) in which you are included;
2) Orginal or certified copies of documents including all passports and proof of citizenship.
3) Your original birth certificate listing both parents names
- Only required if you are a child under the age of 14 or your proof of citizenship is the U.S. passport of a parent in which you are included;
4) Proof of identity (at least 2 of the following):
- a valid passport of any country; or
- a valid driver's license with photograph; or
- a national identity card with photograph; or
- a valid U.S. or State government ID card; or
- a U.S. military ID card.
5) Any foreign passport that you may currently hold, have held, or been included in;
6) Name change documentation:

- Applicants over 14 years of age must sign their own passport application in the presence of the Consular Officer.
- The instructions on pages one and two do not apply if you are applying for a passport in Israel. Follow the instructions on this web page. Do NOT print Form DS-11 on front and back of the same sheet. For tips on completing the DS-11,
8) Two identical passport 5x5 centimeter photographs:
- The Embassy will reject any photographs that do not meet the specified requirements. Vending machine photographs and photographs printed on a home computer are unacceptable. Follow this link to photo requirement specifications;
9) Fees: The application fee for an applicant under the age of 16 is U.S. $85.00; aged 16 and over is $100.00. Accepted payment methods are:
- Cash - either dollars or shekels; or
- Most international credit cards; or
- Travelers Checks in U.S. Dollars.
Note: The Embassy is not authorized to accept personal checks.
10) Envelope:
New passports must be sent back to applicants once they are processed. In order to receive your new passports, you must bring a self-addressed, stamped envelope with at least 16.20 NIS of postage.
11) All children are required to appear at the Embassy when applying for their passports.
12) Both parents of children are required to appear at the Embassy when applying for passports for children under the age of 16.
- If only one parent is able to attend, we can accept the following documents as proof from the other parent:
- A notarized consent Form DS-3053 from the other parent. The consent must include a copy of the U.S. or foreign passport of the parent who is not appearing at the Embassy; or
- Original Death Certificate; or
- Original Court order from granting “sole-custody” (bilahdit) to the parent appearing at the Embassy; or
- Original Birth certificate listing only one parent.
13) For children under 16, parent’s U.S. or Israeli passport.
14) Additional documents may be required depending upon the circumstances of your case.
Method of Application
You are required to apply for the passport in person at this office through appointment only.
Processing Time
An application for a full-validity (ten years for applicants age 16 or over; five years for children under age 16) passport will be sent to the United States for printing with a secure, photo-digitized image of the bearer. Passport times vary depending on workload. Please allow 3-4 weeks for the application to be processed.
